Established in 2014, Foundations was instituted on the principal that building strong foundations through educational support, medical care and awareness as well as community service is vital to the development of a holistic life.
We provide Tutoring, Standardized Test Prep, Early Intervention Developmental Therapy
and In-Home Care for seniors and children.
We understand that finding the right home care agency for your family member will be one of the most important decisions you will ever make. As an alternative to a nursing home for seniors or permanent placement care for children, Foundations is always ready to provide all of that and more for your loved one. Whether it's because you are at work or simply in need of respite care, allow Foundations to be your first choice for caregiving. We offer a wide range of services that will fit your budget from a few hours a week to all day because we are sincerely here for you and your family.
Many seniors can benefit from non-medical home care services. If your loved one struggles with bathing and dressing in the morning, or is having trouble cooking his own meals, Foundations can help. We can also be there to make sure he/she is able to take her necessary prescriptions on time and doesn’t forget a dose, or that the foods in the fridge and the cupboards haven’t expired. These services help your loved one stay independent, with dignity, in their own home, for much longer. We can provide you a trusted non-medical Home Health Aide or Certified Nurses Aide who can even stay the night if necessary. It all depends on your loved one’s individual needs.

Medicaid services allow adults, the elderly and children with disabilities to remain in their home while receiving care. Be sure to discuss your needs with your doctor as the you will need to receive a Medicaid waiver.
The Medicaid waiver allows an aide to come into the home and assist the recipient with personal care and an LPN or RN for skilled care.
Medicaid Agency Directed Services allow Foundations to provide a trained aide to come into the home and provide care to the recipient.
The aide is employed with Foundations and is screened through our Human Resource Department and held to the highest industry standards.
Our private pay services enable you to access important home care services that you might need, even if they are not covered by insurance, Medicare or Medicaid at a very competitive rate.

In addition to the state educational requirements, our therapists have also obtained their NC State CDSA Infant Toddler Certifications.
Services are currently provided in Gaston, Cleveland, Lincoln, Rutherford, Iredell and Cabarrus Counties.

Our Program is socially interactive, engaging, musical and fun. We seek to provide parental support, behavior support and developmental therapies in conjunction with the preschool experience. Our goal is to ensure that a strong foundation of language and learning is implemented through play.
Ages: 2-3 years old
Hours: 9am-12:30pm
Tuition: $50 week ($12.50 a day)

It can be hard when your parents, grandparents or other loved ones reach a point where they struggle with daily living tasks. They still want and actually need to have as much independence as they can, but you worry about what might happen to them when you can’t be there for them due to your own work and family responsibilities. That is why Foundations has a team of companion care professionals who have been trained and thoroughly background checked who can be there to help and watch out for your loved one when you wish you could be, but can’t.
Our elderly companion care team can do many of the things you would do with and for your loved one. Here are some of the things we often help with:
converse, chat with and listen to your loved one. We can discuss current events, books, movies, history, and anything that your family member finds interesting and enjoyable. We can also play games, watch movies, do family history and scrapbooking, read books and do crafts and hobbies together.
We provide your loved one with the personal care needs they deserve.
We
can help with dressing, undressing, bathing, toileting, skin care, light housekeeping, change bed linens, laundry and meal prep.
Everything
you would do for your loved one and we strive to do it with the same amount of love and care.
Being present for wellness needs are important when it comes to ensuring reliable and superior in-home care. We can provide assistance with monitoring your loved one's vital signs including blood pressure, prepare healthy meals, assist with feeding and give prompt medication reminders.
Non-medical home care is a great boost to many seniors and their families, giving them more independence, dignity and comfort in their own home in spite of the various challenges of aging and chronic illness. At Home Personal Care provides non-medical home care services that help seniors stay at home and live the lives they love to live longer. Although your loved one may be having difficulty performing tasks they once did with ease, our team can help them accomplish those daily tasks safely, more easily and without getting worn out. This helps them stay home much longer, and gives your family peace of mind.
Making a complete assessment of your loved one's needs is important for them and honestly more important for you. Effective communication is imperative when providing care for others. A Plan of Care (POC) is the directive for your loved ones daily care. That is why Foundation's resident Nursing Director will provide you with a complete assessment which will give you and the RN an opportunity to ensure that your loved ones needs are being met holistically.
